2) Complete entire SCAMP enrollment application form by clicking link here: docs.google.com/forms/d/e/1FAIpQLSeVCNBERvZd5WlJydXnJ3XvoB7Pv1THFJJoX3CyiminhLOpoQ/viewform?usp=header
3) Indicate plan of action for tuition payment. All tuition must be paid by June 1st.

*There will be a $30 fee on any returned checks.
4) Complete any necessary medication forms or health plans and submit to [email protected] AND bring to Scamp Open House (submit to Health Station):
